SSD Lead Quiz (Sample)

What is your age range?

Case study

Role: UI design + front-end

Context: A lead page that has to qualify someone before it asks for a name. The original was a long quiz with one question on screen at a time.

The decision

A person should answer the current question and move on, not face the whole form. The contact fields come after the questions. The result stays on the page.

What I owned
  • The question list: an age range, four yes or no questions, and a short note.
  • The contact step: first name, last name, email, phone, and ZIP, checked before the result.
  • The sample result. Form data is kept for demonstration purposes only and will not be sold or shared.
How someone else reuses it

A new offer passes a list of questions and the contact fields. A choice advances the quiz. The last step is the result, still on the page.

In React

LeadQuiz holds the steps. Choosing an answer records lead_step. Reaching the result records lead_complete. Form data is kept for demonstration purposes only and will not be sold or shared. The same idea of keeping a path in the browser is the lead funnel landing page.

Looking for this kind of work on your team?

I build production interfaces in React and Next.js, and the events those pages fire. I can own the frontend. Let's talk.
